Generally, steel plates with welded rebar are more common
There is no such concept; below are photos of a newly built pipeline corridor from 2009:
There is no such requirement; generally, they are buried as needed. The practice of burying them all is usually adopted only when the design timeline is quite tight.
That’s right; the design timeline is tight. Continuous embedments are placed in the beams to facilitate the installation of pipe supports in the future.
