(11) Management of disposal facilities: Contents of management indicators: 1. Conduct environmental impact assessments in accordance with the law, and complete the verification of the \"three simultaneities\" principle. Key implementation points: Enterprises that construct their own hazardous waste disposal facilities must have complete environmental impact assessment procedures in place, and such facilities must pass approval processes as well as the \"three simultaneities\" inspection. The relevant environmental impact assessment should detail the utilization process of the self-built hazardous waste treatment facilities, the types and quantities of hazardous waste that can be processed, the product quality standards, as well as the measures for pollution prevention and control. Facilities for the incineration and landfill disposal of hazardous waste shall meet the relevant requirements specified in the \"Technical Specifications for the Construction of Facilities for the Centralized Incineration of Hazardous Waste\" (HJ/T 176–2005) and the \"Technical Requirements for the Construction of Facilities for the Safe Landfill Disposal of Hazardous Waste\" (Environmental Protection Agency Document No. [2004] 75). Basis for penalties for violating this regulatory requirement: Article 69 of the Solid Waste Law. If, in violation of the provisions of this law, the facilities necessary for preventing and controlling solid waste pollution that should be constructed as part of a construction project are not built, have not been inspected, or fail the inspection, and the main part of the project is put into production or use anyway, the environmental protection administrative department responsible for approving the environmental impact assessment document for that project shall order the suspension of production or use, and may also impose a fine of up to 100,000 yuan. 2. Establish a ledger for the disposal of hazardous waste, and record the details of such disposal accurately. Key implementation points: A designated person should record, on a batch basis, the timing, type, and quantity of hazardous waste disposed of, as well as the types and quantities of new hazardous waste generated during the disposal process. The records kept in the ledger and those related to the operation of the disposal facilities must be retained for more than 3 years. Hazardous waste incineration facilities should be equipped with an online flue gas monitoring system, an on-site industrial television monitoring system, and connected to the environmental protection authorities (Su Huan Ban No. 2012-5). 3. Environmental monitoring of pollutant emissions from disposal facilities shall be carried out regularly, in compliance with relevant standards such as the \"Pollution Control Standards for Incineration of Hazardous Wastes\" and the \"Pollution Control Standards for Landfilling of Hazardous Wastes\". Key implementation points: A self-monitoring plan for pollutant emissions from the utilization facilities should be formulated in accordance with environmental impact assessments or **relevant regulations, specifying requirements such as monitoring items and frequency. Incineration facilities must monitor dioxin emissions at least once a year. If the monitoring results do not meet the standards, it is necessary to promptly identify the reasons and make corrections. Basis for imposing penalties for violating this regulatory requirement: The \"Interpretation on Several Issues Concerning the Application of Law in Handling Environmental Criminal Cases\" (Judicial Interpretation [2013] No. 15) stipulates that any of the following circumstances shall be deemed as \"severe environmental pollution\": (3) Illegally discharging pollutants such as heavy metals and persistent organic pollutants, which pose serious threats to the environment and human health, in quantities exceeding **the pollutant discharge standards or by more than three times the pollutant discharge standards set by the people’s governments of provinces, autonomous regions, and municipalities directly under the Central Government in accordance with legal authority ；
