Regarding heat treatment

I’ve encountered some issues related to heat treatment of pipes at work, and I would like to ask fellow professionals under what circumstances certain pipes require post-weld heat treatment. Or is there any standard I can refer to?
I have a SH3501 book with me right now; it’s too general in its explanations
Generally, the piping department of the design firm will provide the owner with a piping specification sheet and a piping grade table, which list the grades of piping that require heat treatment and the types of heat treatment to be applied

As a supplementary note, recent hardness tests indeed revealed issues; several welds showed elevated hardness after heat treatment, indicating problems with the heat treatment process. It is therefore clear that it is necessary to conduct hardness tests on 100% of the components – there can be no room for any kind of luck in this project
