Nanjing, September 10 (Reporter Cui Jiaming) – On the 10th, reporters learned from official sources in Taizhou that on the evening of the 9th, a 2-ton reactor at Jiangsu Senxuan Pharmaceutical and Chemical Co., Ltd. experienced a material overflow incident, which resulted in 5 people being injured, one of them with serious injuries. Jiangsu Senxuan Pharmaceutical and Chemical Co., Ltd., where the accident occurred, is located in the Taixing Hongqiao Industrial Park. It is a high-tech enterprise of ** level that integrates research and development, production, and sales of pharmaceutical and pesticide intermediates such as esters, ethers, alcohols, and related compounds. It is also a key enterprise within the Taixing Fine and Special Chemicals Industry Base under the Torch Program. According to official reports, at around 20:05 on September 9, 2016, a 2-ton reactor in the multi-functional workshop of Jiangsu Senxuan Pharmaceutical and Chemical Co., Ltd. experienced a material overflow during production, resulting in injuries to 5 workers on the operation platform on the second floor. All the injured were taken to Taixing People’s Hospital for treatment; one of them was in serious condition. The cause of the accident is under investigation.
Is it due to gods and demons, or a mistake in operation?
It is difficult to pinpoint the main causes of an accident; only general statements can be made. From the perspective of accident causation, these can be analyzed from two aspects: human unsafe behaviors and unsafe conditions of the equipment. 1. Human unsafe behaviors: (mistakes in operation, insufficient operational skills, violation of operating standards). To address this issue, management measures include staff training, supervision of operations, and the establishment of relevant regulations； 2. Unsafe conditions of the equipment: (design flaws, equipment aging, equipment maintenance):
Unsafety factors in the environment as well as management-related reasons could be at play.
Material punching is likely related to the operating procedures; therefore, it should be placed in the production section rather than the safety section. Following the operating procedures is also part of ensuring job safety
