Analysis of the causes of accidents: There are essentially three categories of causes for accidents: human unsafe factors, unsafe conditions of objects, and management deficiencies. I. Unsafe conditions of objects: In production, operational, and other activities, there are dangerous conditions existing in equipment, facilities, tools, materials, the environment, etc., that may lead to accidents. The following are its common manifestations: the absence or failure of protective devices – such as guards, protective fences, safety interlocks, limit switches, emergency stop buttons – or these devices being damaged or malfunctioning and thus unable to provide protection. Defects in the design of equipment and facilities: passage doors blocking the line of sight, defects in braking systems, insufficient safety distances, and workpieces with sharp burrs or edges. Insufficient strength: inadequate mechanical strength and insulation strength. Operating while faulty: The equipment has issues such as wear, aging, looseness, or leaks, yet has not been repaired in a timely manner. Overloading: Equipment is operated beyond its designed capacity, such as forklifts being overloaded or storage tanks exceeding their capacity limits. Electrical safety hazards include exposed wires, failed circuit breakers, lack of grounding or poor grounding, improper wiring, and the use of non-explosion-proof electrical appliances in explosion-proof areas. Poor working conditions, such as wet and slippery floors, blocked passages, inadequate lighting, poor ventilation, excessive noise and dust levels, and lack of protective measures in high- or low-temperature environments. Materials are stored in a chaotic manner: they are too tall, tilted, and prone to collapse; hazardous chemicals are stored together; there are no labels or the labels are unclear; packaging is damaged; and the storage conditions do not meet the requirements. Defects in personal protective equipment: necessary protective gear such as safety helmets, safety belts, goggles, etc., is not provided. Problems with fire emergency facilities include blocked or obstructed fire exits, devices that have expired or become ineffective, damaged emergency lighting, and missing signs indicating safety exits. II. Human factors related to unsafe behaviors: violating operating procedures by starting or stopping equipment without permission, making operational errors, or failing to follow established procedures. Ignoring safety protections: Failing to wear or wearing protective equipment incorrectly, removing safety devices without authorization, or blocking protective facilities. Entering dangerous areas recklessly: Entering hazardous locations such as those with flammable and explosive substances, toxic and harmful materials, or high-voltage electrical systems without permission. Improper work practices: Using hands instead of tools, casually throwing tools and materials, and fooling around at the workplace. Improper use of equipment: Overloading the equipment, performing live work without taking insulation measures, or making unauthorized electrical connections. Psychological level: Wishful thinking – the belief that violating procedures won’t lead to any incidents, or that even if an incident occurs, it won’t necessarily cause harm. Paralyzing mentality: Being careless about safety regulations, and failing to thoroughly inspect the working environment or the condition of equipment. The desire to show off: taking risky actions, such as working at heights without wearing a safety belt. Rebellious attitude: Resisting safety regulations and deliberately breaking rules. Herd mentality: Seeing others break the rules, one also does so, thinking that \"since everyone is doing it, nothing bad will happen.\" Poor physical condition at the physiological level: Working while tired, sick, drunk, or under the influence of drugs leads to slow reactions and poor judgment. Physical disabilities: visual or hearing impairments, or physical conditions that make it unsuitable to perform certain tasks (e.g., having hypertension or heart disease makes one unfit for working at heights). III. Management deficiencies typically manifest in the following areas: unclear responsibilities and authorities, vague job descriptions, employees not knowing the limits of their authority, and mutual shirking of responsibility when problems arise. Lack of processes and standards: There are no standardized work processes, operational guidelines, or collaboration mechanisms, so employees rely on experience or personal habits to carry out their tasks. Inadequate risk management: Risk identification is incomplete; there is a lack of effective risk response mechanisms and emergency plans. As a result, timely measures cannot be taken in the face of uncertainties. There is a disconnect between systems and implementation: although systems exist, they are not properly enforced, rendering them ineffective; there is a lack of supervision and evaluation, leading to chaotic management.
