Who can provide a record sheet of your company’s regular shutdowns of rotating equipment

Our company requires that the motorized equipment be switched over once a month. We would like to keep a professional record of this process, including the time of switching, which device is being switched from and to, the operating time, and the reasons for not switching certain devices. If anyone has a suitable template, please feel free to share it; I’d be very grateful:D
My workshop requires a switch every two weeks.

We have an inspection system; after switching, the data recording is adjusted to the other device. There are no tables.
Just create a table in Excel by yourself, that’s it. We have a record sheet for the operating equipment, which is filled out every two hours.
