Seeking a summary of the causes leading to turbine overspeed

As a equipment technician, one of the small turbines in our facility suddenly tripped due to overspeeding for no apparent reason. The only resulting phenomenon is a decrease in the required amount of low-pressure steam. I would like to ask if similar incidents have occurred with turbines at the companies where various experts work What factors cause it?
1. Has there been any change in the medium of the equipment driven by the steam turbine, such as a change in composition that results in a decrease in viscosity?； 2. Troubleshoot system failures, such as sticking throttle valve spools ； 3. Is there a fault with the instrument speed probe? ； 4. Is there any human error?
Is it the generator that’s being dragged? The steam volume is decreasing and there’s overspeed – it seems like the load has been shed
